Do you accept insurance?

Yes, I am a network provider on most insurance plans.  If you will provide detailed information about your plan we will determine if I am on their provider panel.

What is your cancellation policy?

At least 24 hours notice is required for cancellation or rescheduling appointments.  Appointments that are missed are charged in full.
